  • Provider has time to address your concerns, provide prompt follow-ups, and share the latest, evidence-based wellness and health maintenance advice.  Our providers typically spend a minimum of 40 minutes for each in-home encounter.

  • Assurance of weekday appointments within 24-72 hrs to address changes in health status due to providers having smaller patient populations. Appointment using telephone, telehealth, and in person visits.

  • Individual care services with increased communication access to providers via telephone, email, and patient portal outside of normal business hours.

  • Dedicated care plan management and care coordination to assure seamless navigation through the healthcare environment.

    • Annual Flu shot administration in home

    • Consultation for home safety assessment 

    • Coordination of home care and durable medical equipment needs

    • Guidance for assisted living, memory and dementia care placement

    • Completion of Long-Term Care insurance paperwork, Assisted Living Admission paperwork, FMLA paperwork

  • Arrange for full diagnostic testing services in home to minimize emergency room services.

    • Phlebotomy services in home (Annual included in fees, periodic testing at additional $60 cost per lab draw) with prompt turnaround time.

    • Lab tests in home such as Covid Testing, respiratory panel, UTI panel in home through provider.

    • Access for mobile imaging such as x-ray, ultrasound or venous/arterial doppler studies, and EKG.

  • Medication Management

    • Medication refill requests can be sent outside regular appointments if needed, with the exception of any controlled substances and if requesting three or more refills an appointment must be made

    • Completion for Prior-authorizations for medications

  • Specialty care coordination and phone conferences with family/patients as needed

  • Travel time for the provider to your home

Membership Pricing:


  • Initial/Annual Membership fee: $500 per person, $800 per couples, to be charged upon joining and annually

  • Providers will gauge frequency of visits at initial visit and will be assessed on an ongoing basis.

  • Monthly fees-to be billed at the first of each month: $250/month

  • Please note there is a 3% service charge for credit card payments.

  • Fees will be billed to credit card authorized on file.

  • Fees are subject to change annually.


Medicare billing


  • Medicare and supplemental insurance providers will be billed for covered services to include: 

    • Visits - in home, telephone, telehealth

    • Laboratory services

    • Diagnostic imaging services

    • Home Health care such as nursing and the following therapies; physical, occupational, and speech. 

    • Hospice and Palliative Care
